General terms and conditions

• In an extensive intake interview, I will ask you questions about your health and personal situation, among other things. You remain responsible for providing all information that may be relevant to the treatment plan as fully and truthfully as possible. In case of pain complaints, a doctor should be consulted first. I will always advise you to consult a doctor if I deem it advisable.

Please read the treatment agreement and intake form carefully and fill them in truthfully. You will return these forms prior to the first appointment. If, for additional privacy reasons, you wish to protect the forms with a password, you should inform us of this yourself.

• You remain responsible for the progress of the therapy. Just as you remain responsible for what you do with the knowledge and experiences gained.

• Should you be dissatisfied, please let me know. Should you remain dissatisfied despite our joint consultation, you can contact the NBVH and appeal to a complaints officer as prescribed in the Wkkgz (Quality, Complaints and Disputes Care Act). MV-inbeeld is also, following this law, affiliated to the SCAG disputes body. I am registered in the Hbo-Register Beroepsbeoefenaren in de Complementaire Zorg. And I am subject to disciplinary law provided by the TCZ (Stichting Tuchtrecht Complementaire Zorg).

• Appointments made can be cancelled by phone up to 24 hours before the session. If this deadline is exceeded, 50% of the cost of the session will be charged. In case you unexpectedly fall ill and cannot be physically present, there is the opportunity to cancel the session up to two hours before the session in consultation with me or to continue the consultation online via Teams. If I am unable to attend, you will be notified as soon as possible. I will then reschedule as soon as possible.

• The cost of treatment should be paid by tap or cash payment after the consultation. If payment cannot be made immediately after the consultation, pay the amount due within 14 days of the invoice date. If you forget to pay, you will receive a payment reminder. If you do not pay within 14 days after the date of a payment reminder, I am entitled to take collection measures. If an invoice is not paid on time, it is not possible to make a new appointment. The invoice will be sent by e-mail. If, for additional privacy reasons, you would like to receive the invoice with a password to view the file, you should make this known yourself.

• In addition, for persons who have come to me(n site) through any of the following websites: therapypsychologist.com, emdrtherapie.net, anxietyphobiatherapie.com, hypnotherapists.org, I have a partnership with the company Evenio (which operates the above websites). To ensure proper security and confidentiality, a processor agreement has been concluded with Evenio. In terms of processing personal data, I am the processor and Erwin Coenen of Evenio is a sub-processor (terms from the AVG legislation). For individuals who have come into contact with me through the above websites, I pay a premium to Evenio for the mediation provided. Erwin Coenen provides a mail check to verify that I have received all messages and that I have passed on whether or not someone has come for an appointment, so that it is clear how much premium can be billed by Evenio to the me. Content, private matters and date of birth and so on, i.e. files, are of course not shared with Evenio (see privacy statement).
